Intuit GoPayment comes to the UK, renames itself Intuit Pay

In a bid to catch mobile payment rival Square napping, Intuit GoPayment is leaping across the pond to begin an assault on the UK. It's certainly putting the effort in, having had to rework its hardware to be compatible with Chip-and-Pin and being forced, for legal reasons, to drop the "Go" and "ment" parts of its name. The service is currently in a limited trial before being rolled out to switched-on stallholders and itinerant fraudsters (we kid) nationwide.

Why Customer Loyalty Starts With Company Culture: Q&A With Vala ...

As chief customer officer at Enterasys, an Andover, Mass.-based enterprise networking company, Vala Afshar has piled up 81,800 tweets ? about 130 per day ? ?since signing up for an account in early 2011. His Twitter feed reads like a constant stream of daily affirmations and insights about management and marketing.?But for Afshar, who doubles as Enterasys?s marketing chief, being a social junkie is no gimmick. ?He?s using the platform, he says, to create a culture of transparency, which leads to engaged frontline employees, and, in return, top-notch customer service. It?s also the backbone of what he calls a ?social business.? The co-author of The Pursuit of Social Business?talked to The SmartVan about how field service organizations stand to gain by embracing the same idea.

The SmartVan: What is a social business?

Vala Afshar: A social business is not about social media or social tech ? it?s about a mindset of inclusiveness and shared compatibility. Not one of us is as smart of all of us. You have to have a collaborative mindset to deliver solutions that matter to customers. And happy employees translate to happy customers. The reason for our success at Enterasys has been our culture, our transparency. We believe communication improves culture.?

How exactly have you implemented a social culture at Enterasys, and what have been the results??

In 2010 Salesforce introduced Chatter, a social technology for the enterprise. We were one of the first companies to implement it. Employees connect to you and follow you just like in Facebook. We offer Chatter in our customer service and support space. We created tech groups: a wireless group, a routing group, etc. Field personnel and engineering personnel can be part of these groups. If a field worker has a question about wireless, they can chat about wireless and about a solution in real time. By using this ecosystem, we are able to improve contact culture. We can publish a response and solution to any scenario. Social collaboration improves delivery. This has led to $1 million a year in terms of cost savings. There is less need to hire additional staff and there are efficiency gains in processing cases.

How does social business relate to field service?

In the service world, companies are challenged in terms of a new complexity of technology. There?s a shift that?s happening where it?s less and less about the product and more and more about solutions. How do you solve the problem? The support department is no longer siloed ? it is now a process. You have to connect the engineer with the field worker and IT department. You have to connect frontline employees with service personnel. You need more and more frontline employees to have access to information in real time.

Studies show that customer loyalty and commitment is no longer about customer satisfaction. The execution of service is linked to loyalty. The faster you deliver a solution the more likely you?ll have customer loyalty. Social has helped us improve this business agility.

How do you create a culture among a dispersed workforce??

There are critical success factors for social business satisfaction and superior customer support: First there is culture. For culture you need people. You need to hire for attitude and train for aptitude. If person has passion to serve, you are winning as a business. The company?s brand is the frontline employee. Trust the people you hire and trust them to do the work. A social mindset is where there is clear transparency. Then there is process. If you are process-heavy, you are not a social business. Social business is about agility. It?s counterintuitive ? you have to shift from command-and-control to collaboration and co-creation. Define a lean process first, then find the tech to support it.

So how does tech fit into all of this?

Tech shrinks the ecosystem. After this interview, I?m going to get on Chatter and say I had great talk with The SmartVan. My 500 followers will see it and some will chat back. I will get on Twitter and other folks from Twitter will see it. Over time people will ? hopefully ? find you interesting and compelling. Tech is a pull that gets us more connected. Regardless of whether you are under one roof or in the field, communication improves culture. Mangers have to be interested in their employees? work. It means you have to stretch yourself. Collaboration is not easy. Social tech helps you connect and have a better understanding of field personnel. It helps overcome field elements in terms of transparency and communication.

What?s the future for service organizations?

Companies that are not social and not embracing mobile are going to put the field service profession in real trouble. Social and mobile is about a lifestyle now. The customer is driving it and service organizations need to adapt. Leadership is conversation and you have to be where the conversation is. Customers spend time on social networks and you want to deliver messages where you think the user will be.?My customers can send a tweet on Twitter asking for advice and I can respond. My guess is three to four years from now, we will all be collaborating in real-time video.?

We are also going to see more machines talking to social networks. When a component fails, the machine sends a social message on Facebook, Twitter or Chatter to the closest service station, along with the customer. That?s the future of service delivery ? social machines communicating to call centers and field personnel in real time. The future of social business is not just people, it?s people and products.?

Basis Band fitness tracker finally goes on sale priced at $199, mobile apps still MIA

Remember the Basis Band, that fitness-tracking wristband that was supposed to come out earlier this year? Well, the company obviously missed its target for an "early 2012" launch, but it's been busy refining the product and publishing intermittent blog posts to let people know how the device has been shaping up. Well, at last, it's ready for prime time: the band is on sale today, priced at $199, as originally planned.

In case you're just hearing about Basis for the first time, here's a quick recap: unlike Nike Fuelband and other devices aimed at athletes, Basis is attempting to lure in more mainstream users with the ability to set goals and get rewarded for staying on track. (Other devices, including the new Jawbone Up, do this too.) What's interesting, though, is that for a device that claims to target the everyman over the nerd it actually collects an impressive amount of data. For instance, the "Patterns" view in the web console will show not just how many steps you take throughout the day, but which hours of the day you were most active. The band, meanwhile, tracks sweat output, heat dissipation, blood flow and heart rate -- something we can't say about all those other fitness trackers. As for the UI, the site itself is visually pleasing, though some mobile apps would also be nice. (A Basis rep says they're working on it.)

If anything, the product's simplicity comes from the hardware: like other new fitness trackers, it uses Bluetooth 4.0 to transmit all your data to the cloud so that you don't have to sync over USB. Basically, between that and those various sensors, the band should theoretically detect when you're sleeping or exercising so that you don't have to switch modes or enter any information manually. It's worth noting, though, that other fitness trackers mainly stumble when it comes to logging food intake -- something sensors can't detect -- and that's not even something the Basis band covers right now. In other words, it's easy to praise a device for working automatically when it completely bypasses the whole diet piece of the wellness equation. According to Basis, research suggests people don't really use these food-logging tools anyway. We can believe that, actually, though for $199 it would be nice to at least have the option, no? That's another thing: at two hundred bucks this is considerably pricier than competing devices, including the Up band ($129) and the new Fitbit One ($100). Even so, the user experience is everything, isn't it? If you've ever been food-judged by Up, or if the Basis band really is as low-maintenance as it seems, it could still be worth a gander.

Chevy launches iPhone Siri support for Spark, Sonic vehicles, aims to keep your Eyes Free

If you're looking at getting a 2013 Chevrolet Spark or Sonic LTZ / RS, you'll be able to drag Siri along for the ride, provided that you have an iPhone 4S or later running iOS 6, according to GM. It'll pair with Chevy's MyLink entertainment system using Bluetooth, ensuring you avoid distractions via Apple's new Eyes Free mode that lets you disable the smartphone's screen via a steering wheel button. Using voice-activated commands, you'll be able to prompt the system to make hands-free calls, play songs from your iTunes library, listen to and compose text messages and access calendar appointments. If you ask a question that requires a web page to be displayed, however, Siri will decline to answer -- ensuring your eyes stay where they should.

The automaker also announced an upcoming version of MyLink for the Chevrolet Impala that will let buyers "skin" the instrument panel with four different themes: Edge and Velocity for the younger set, and Contemporary or Main Street aimed at "busy moms" or business types. Oil debate heats up, house adjourned prematurely

Parliament has been adjourned for short while after the MPs failed to agree on the procedure of the debate on the oil bill.

The speaker Rebecca Kadaga has been forced to adjourn the session for 15 minutes after a section of MPs opposed to the re-committal of article 9 of the bill of the oil bill started chanting and shouting, over shadowing the entire debate.

They have also disagreed on the whether or not to vote on the issue, prompting the speaker to halt the process temporarily.

Parliament is today expected to debate and pass the oil bill amid criticism from a section of MPs, the civil society and religious leaders who want the bill amended before it can be passed.

The MPs opposed to the re-committal of article 9 of the bill that gives the minister powers to grant and revoke licenses and negotiating the agreements with the oil companies.

The MPs want those powers to be given to the national petroleum authority instead of the minister whom they say is not a technical person.

Study points to potential new therapies for cancer and other diseases

ScienceDaily (Nov. 27, 2012) ? Researchers at The Scripps Research Institute (TRSI) are fueling the future of cancer treatment by improving a powerful tool in disease defense: the body's immune system. By revealing a novel but widespread cell signaling process, the scientists may have found a way to manipulate an important component of the immune system into more effectively fighting disease.

The study, recently published online ahead of print by the journal Blood, shows that disabling a particular enzyme, called ItpkB, in mice improves the function of a type of immune cell called Natural Killer cells.

"This is an exciting finding because it could possibly lead to the development of drugs that improve Natural Killer cell function," said TSRI Associate Professor Karsten Sauer, PhD, who led the study. "Natural Killer cells have gained clinical interest as innovative biological therapeutics for certain cancers and also in certain infectious diseases."

The Body's 'SWAT Team'

Natural Killer cells patrol the body and detect characteristic alterations on the surface of cancer cells or virus-infected cells. Through a complicated and little understood signaling machinery -- a domino effect of molecular reactions in a cell that ultimately produces a certain signal -- Natural Killer cells then destroy such "stressed" cells.

Compared to other types of immune cells, Natural Killer cells kill these cells quickly. This makes Natural Killer cells important early responders of the immune system. Not surprisingly, researchers have explored engaging this "SWAT team" of the body therapeutically, particularly in blood cancers.

However, to date, the therapeutic efficacy of Natural Killer cells has been limited. "A key bottleneck is our limited understanding of signaling mechanisms that dampen Natural Killer cell function," Sauer said.

Sauer and colleagues' new research reveals crucial details of this puzzle.

A Way to Prime the Attack

The Sauer group had previously identified ItpkB as a key regulator of immune function. ItpkB acts primarily by producing IP4, a small molecule messenger that controls the functions of various other important signaling molecules. IP4 can improve or inhibit signaling depending on the cell type in which it is produced.

The new research showed that Natural Killer cells from mice lacking ItpkB show elevated signaling and function better than Natural Killer cells that have the enzyme. As a result, mice lacking ItpkB are more effective than mice expressing ItpkB in attacking cells that display characteristic surface changes of cancer cells.

"The enzyme ItpkB has unique features that facilitate its highly specific inhibition by small molecules," said Sauer. "Our findings suggest that such compounds could possibly be used to improve Natural Killer cell function therapeutically. If successful, this could overcome a bottleneck and engage the body's SWAT team to fight cancer."

In addition to Sauer, Eugene Park of Washington University School of Medicine was a co-first author of the study, "Inositol Tetrakisphosphate Limits NK Cell Effector Functions by Controlling Phosphoinositide 3-Kinase Signaling." Other authors include Sabine Siegemund, Luise Sternberg and Stephanie Rigaud of TSRI and Anthony R. French, Joseph A. Wahle, A. Helena Jonsson, Wayne M. Yokoyama and co-corresponding author Yina H. Huang of Washington University School of Medicine.

The research was supported by grants from the National Institutes of Health (AI070845, GM088647, AI089805 and AI007606), The Leukemia and Lymphoma Society Scholar Award 1440-11 and Deutsche Forschungsgemeinschaft fellowship SI 1547/1-1.

ISTANBUL -- An autopsy on the exhumed body late President Turgut Ozal, who led Turkey out of military rule in the 1980s, has revealed evidence of poisoning, a Turkish newspaper reported Monday.

There had long been rumors that Ozal, who died of heart failure in 1993 at the age of 65, was murdered by militants of the "deep state" -- a shadowy nationalist strain within the Turkish establishment of the day. He had angered some with his efforts to end the Kurdish conflict and survived an assassination bid in 1988.

His body, dug up last month on the orders of prosecutors investigating suspicions of foul play in his death, contained the banned insecticide DDT and the related compound DDE at ten times the normal level, Today's Zaman cited sources from the state Forensic Medicine Institute as saying.

"Ozal was most likely poisoned with four separate substances," the paper reported the sources as saying, also naming the toxic metal cadmium and the radioactive elements americium and polonium as substances found in Ozal's remains.

Forensic institute officials declined to comment.

Dominant figure in Turkish politics in 1980s
Ozal, whose economic reforms easing the grip of the state on business helped shape modern Turkey, was in poor health. After undergoing a triple heart bypass operation in the United States in 1987, he kept up a grueling schedule and remained overweight until he died.

His moves to end a Kurdish insurgency and create a Turkic union with central Asian states have been cited as motives for would-be enemies in "deep state," in which security establishment figures and criminal elements colluded.

It was Turkey's military leaders who appointed him as a minister after a period of military rule following a 1980 coup.

He went on to dominate Turkish politics as prime minister from 1983 to 1989. Parliament then elected him president, but those close to him believe his reform efforts displeased some in the security establishment.

While prime minister, Ozal survived an assassination attempt by a right-wing gunman in 1988 when he was shot at a party congress, injuring a finger.

Frequent coups
Turkish political history has been littered with military coups, alleged anti-government plots and extra-judicial killings. A court is currently trying hundreds of people suspected of links to a nationalist underground network known as "Ergenekon" accused of plotting to overthrow the government.

A media report at the start of November said Ozal's autopsy had revealed high levels of the pesticide strychnine, but the ATK subsequently denied the report.

The head of the Forensic Medicine Institute?has said it aims to complete its work in December and that its report would be handed over to prosecutors.

Civil rights leader Lawrence Guyot dies at 73

WASHINGTON (AP) ? Lawrence Guyot, a civil rights leader who survived jailhouse beatings in the Deep South in the 1960s and went on to encourage generations to get involved, has died. He was 73.

Guyot had a history of heart problems and suffered from diabetes, and died at home in Mount Rainier, Md., his daughter Julie Guyot-Diangone said late Saturday. She said he died sometime Thursday night; other media reported he passed away Friday.

A Mississippi native, Guyot (pronounced GHEE-ott) worked for the Student Nonviolent Coordinating Committee and served as director of the 1964 Freedom Summer Project, which brought thousands of young people to the state to register blacks to vote despite a history of violence and intimidation by authorities. He also chaired the Mississippi Freedom Democratic Party, which sought to have blacks included among the state's delegates to the 1964 Democratic National Convention. The bid was rejected, but another civil rights activist, Fannie Lou Hamer, addressed the convention during a nationally televised appearance.

Guyot was severely beaten several times, including at the notorious Mississippi State Penitentiary known as Parchman Farm. He continued to speak on voting rights until his death, including encouraging people to cast ballots for President Barack Obama.

"He was a civil rights field worker right up to the end," Guyot-Diangone said.

Guyot participated in the 40th anniversary of the Freedom Summer Project to make sure a new generation could learn about the civil rights movement.

"There is nothing like having risked your life with people over something immensely important to you," he told The Clarion-Ledger in 2004. "As Churchill said, there's nothing more exhilarating than to have been shot at ? and missed."

His daughter said she recently saw him on a bus encouraging people to register to vote and asking about their political views. She said he was an early backer of gay marriage, noting that when he married a white woman, interracial marriage was illegal in some states. He met his wife Monica while they both worked for racial equality.

"He followed justice," his daughter said. "He followed what was consistent with his values, not what was fashionable. He just pushed people along with him."

Susan Glisson, executive director of the William Winter Institute for Racial Reconciliation at the University of Mississippi, called Guyot "a towering figure, a real warrior for freedom and justice."

"He loved to mentor young people. That's how I met him," she said.

When she attended Ole Miss, students reached out to civil rights activists and Guyot responded.

"He was very opinionated," she said. "But always ? he always backed up his opinions with detailed facts. He always pushed you to think more deeply and to be more strategic. It could be long days of debate about the way forward. But once the path was set, there was nobody more committed to the path."

Glisson said Guyot's efforts helped lay the groundwork for the Voting Rights Act of 1965.

"Mississippi has more black elected officials than any other state in the country, and that's a direct tribute to his work," she said.

Guyot was born in Pass Christian, Miss., on July 17, 1939. He became active in civil rights while attending Tougaloo College in Mississippi, and graduated in 1963. Guyot received a law degree in 1971 from Rutgers University, and then moved to Washington, where he worked to elect fellow Mississippian and civil rights activist Marion Barry as mayor in 1978.

"When he came to Washington, he continued his revolutionary zeal," Barry told The Washington Post on Friday. "He was always busy working for the people."

Guyot worked for the District of Columbia government in various capacities and as a neighborhood advisory commissioner.

D.C. Delegate Eleanor Holmes Norton told The Post in 2007 that she first met Guyot within days of his beating at a jail in Winona, Miss. "Because of Larry Guyot, I understood what it meant to live with terror and to walk straight into it," she told the newspaper. On Friday, she called Guyot "an unsung hero" of the civil rights movement.

"Very few Mississippians were willing to risk their lives at that time," she said. "But Guyot did."

In recent months, his daughter said he was concerned about what he said were Republican efforts to limit access to the polls. As his health was failing, he voted early because he wanted to make sure his vote was counted, he told the AFRO newspaper.

Scientists rush to save iconic manta rays

RAJA AMPAT, Indonesia ?They?ve been described by one scientist as ?pandas of the ocean.?

?They?re such an iconic species, beloved by divers,? said Andrea Marshall, director of the?Marine Megafauna Foundation, who came up with the description during an interview with NBC News. ?They?re just amazing.??

Unlikely as it might seem, the panda and the manta ray have a lot in common.

Just as scientists still haven?t been able to confirm the number of pandas in the wild, they also have no idea how many manta rays exist.

?Globally we don?t know how many manta rays there are,? said Guy Stevens, director of the U.K.-based?Manta Trust,?whose research is largely based around manta populations in the Maldives.

But -- again, like the panda -- scientists think it?s a small population.

?If they?re lucky, (manta rays) have two pups (over several years).?That?s a very low reproductive rate, especially compared to your average fish,? said Dr. Heidi Dewar, a biologist at the?Southwest Fisheries Science Center, part of NOAA.

Anecdotal evidence suggests mantas are under threat, and China may be a major reason for it.

Manta rays are vulnerable on two fronts: as bycatch ? getting caught in industrial fishing nets targeting different types of tuna ? and, increasingly, because of traditional Chinese medicine, or TCM.?

Manta rays are abundant in the waters around Raja Ampat, eastern Indonesia.

Manta rays are harvested for their gill rakers, which allow the fish to filter food from water.? Some Chinese believe they have healing properties or are good at cleaning out toxins. One Chinese-language?website claims gill rakers enhance the immune system, promote blood circulation and aid in the treatment of cancer, skin disease and infertility.

?It?s just cartilage,? said Dewar, echoing skepticism expressed by many scientists.

Medicinal fad?
Conservationists say manta rays aren?t even considered ?traditional? medicine and argue no reference to the animal can be found in TCM books dating back a century. But with rising incomes that enable Chinese consumers to readily adopt medicinal fads, the impact on manta rays has accelerated over the past 10 to 15 years.?

?A lot of it is completely unrecorded,? said Stevens, who worked on a project founded by Shark Savers and WildAid to document the scope of gill-raker harvesting.?

Researchers looked at the location, value and species involved. ?It does seem the majority of all of those gills that are being traded are ending up in China,? Stevens said.

The conclusion, published in a report called?Manta Ray of Hope, found that roughly 3,400 manta rays and 94,000 mobulas?(related to the manta ray family) are caught each year, but the numbers reflect only reported catches. ?Unreported and subsistence fisheries will mean true landings are much higher,? the report said.?

Visits to random TCM shops in Beijing and Shanghai turned up no gill rakers. In fact, a veteran pharmacist at Tongrentang, a long-established purveyor of traditional Chinese and herbal medicines, said she had never heard of manta rays being used this way.

But the Manta Ray of Hope report estimates a mature ocean manta could yield up to 15 pounds of dried gills that can bring in as much as $230 a pound in a market in China.?

Marshall said she has noticed an uptick in manta fishing. ?I?ve been (in Mozambique) in the last decade ? and we?ve seen an 87 percent decline in the population because of the fishing.??

Unlike many shippers, Chinese merchants who transport cheap products from the mainland for export to Africa ?want to fill [their unloaded cargo vessels] with resources wherever they go.? In Africa, they fill it up with wood, fish or shark?s fin,? she said.??They?ll go out to the local fisheries along the coastline and scout for these products.??

The scientist has spoken to members of local communities, who say the Chinese offer ?new nets, new lines, new hooks.?(The Chinese traders) say to them, ?If you get the sharks or the mantas or the turtles, you get all the meat. You can keep all the meat. You just sell us the things you don?t normally eat.'??

Protecting a ?threatened? species
Mantas were listed last year as ?threatened? under the international Convention on the Conservation of Migratory Species.

The?International Union for the Conservation of Nature?has classified the manta ray as ?vulnerable? to extinction.?

Chinese scientists have also weighed in.?

?In the last two years, we have conducted evaluations of the manta ray and submitted a recommendation to the government to list it as a protected species,? said Professor Wang Yanmin from Shandong University?s Marine College.

?There is no regulation for protecting the manta ray so sales of mantas are not illegal,? said Feng Yongfeng, founder of Green Beagle, a group that promotes environmental protection.

Groups like Manta Trust are focusing on getting manta rays listed by the Convention on International Trade in Endangered Species (CITES). But scientists have their work cut out for them.?

?It?s very difficult to get listed on CITES.? They ask for a lot of detail that is difficult to pin down,? said Marshall. ??Maybe in the terrestrial world, biologists can provide those kinds of details. ?When you?re talking about the megafauna [or large marine species] world, it?s very difficult.??

Marshall ? who discovered a second type of manta ray in 2008 and is in the process of identifying a third -- acknowledges little is known about them.

I?d love to follow an animal to find out how they spend their time,? said Stevens. ?The tagging gives you small glimpses of them.??

Two dive instructors at the Misool Eco Resort and Conservation Center in Raja Ampat have uncovered a revenue stream to offset research costs: tourism.

?One manta ray can raise $1 million (U.S. dollars) in tourism income over its lifetime,? said Rebecca Pilkington-Vincett, citing a figure contained in the Manta Ray of Hope?report.

With the blessing of the resort, Pilkington-Vincett and Calvin Beale launched a research project off the surrounding reefs. ?

?I think it?s fascinating,? said Dewar of the Southwest Fisheries Science Center, ?that there is such a large and amazing creature that has so many mysteries attached to it.??

Relatives of people dying suddenly from heart problems have ...

Relatives of young people who have died suddenly from a heart-related problem are at greatly increased risk of developing cardiovascular disease according to a study published online November 13 in the European Heart Journal [1].

The authors of the study say these findings have two major implications; firstly, they strongly support the fact that autopsies should always be performed in cases of young sudden unexplained death; and secondly, since the involved diseases are all treatable if identified in time, the findings suggest that close relatives of these victims should be screened to detect those who would benefit from preventive treatment.

Researchers identified 470 victims of sudden cardiac death (SCD), aged between 1-35, in Denmark between 2000-2006 and followed their first and second-degree relatives for up to 11 years. They found that relatives who were younger than 35 had a three-fold increased risk of any cardiovascular disease (CVD), a six-fold increased risk of ischaemic heart disease (reduced blood supply to the heart) and a more than 10-fold increased risk of cardiomyopathies (damaged or weakened heart muscle) and ventricular arrhythmias (potentially fatal disturbances in the rhythm of the heartbeat) when compared to the general population.

For young, first-degree relatives of SCD victims, the risks were even higher: a six-fold increase in ischaemic heart disease and a 20-fold increase in cardiomyopathy and ventricular arrhythmia.

Dr Mattis Flyvholm Ranthe (MD), a PhD student and research fellow at the Department of Epidemiology Research, Statens Serum Institut, Copenhagen, Denmark, who is the first author of the report published today, said: ?The most interesting findings were that risk of CVD overall was increased no matter what subgroups of SCD victims and relatives we examined, and that young (under 35 years) first-degree relatives overall had a four-fold increase in risk of any CVD, and nearly 20-fold increases in risk of cardiomyopathy and ventricular arrhythmia. Finding a risk factor associated with a four-fold or greater increase in CVD risk is rare!?

SCD is defined as sudden, unexpected death due to natural unknown or cardiac causes, with an acute change in cardiovascular status within one hour of death or, in unwitnessed cases, in a person last seen functioning normally within 24 hours of being found dead. In young people it is often caused by undiagnosed heart problems that may be hereditary, indicating that there may be genetic mutations involved in the condition.

Dr Ranthe said: ?If sudden cardiac death has genetic causes then this would suggest that relatives of young SCD victims are at greater risk of heart disease than the general population and would benefit from screening to identify those at risk so that they could be given appropriate preventive treatment. This could save a significant number of lives. However, as far as we are aware, there have been no population-based findings of this kind before, and current knowledge is limited and based on smaller descriptive studies.

?Our results show SCD, or the underlying heart problems, has a large hereditary component, and that relatives, particularly young, first-degree relatives, are at much greater risk of developing heart conditions compared with the general population. Family members of young SCD victims should be offered comprehensive and systematic screening, with the focus on the youngest and nearest relatives.

?Most of the cardiovascular diseases leading to sudden cardiac death are treatable, if diagnosed in time, before a fatal event. Our results reinforce the concept of screening as a rational ?tool? to identify such cardiovascular diseases in family members at risk, and thereby possibly prevent future sudden cardiac deaths. However, our findings do not suggest blanket screening of, for example, asymptomatic second-degree relatives of SCD victims, although cascade screening might trigger screening of more distant relatives after a CVD finding in a first-degree relative.?

An autopsy was performed in 67% (314) of the SCDs, and the autopsies confirmed a heart-related cause of death in 178 cases, with 136 remaining unexplained. Dr Ranthe said that conducting autopsies in such circumstances was important because having the cause of death correctly identified was vital information for the relatives, and also for guiding screening initiatives.

?Our study, along with findings from previous descriptive studies, suggests that screening in relatives of SCD victims should be driven by the suspected cause of death, based on autopsy findings and description of the circumstances leading up to the SCD. We would like to emphasise the importance of autopsy in guiding such screening initiatives; public awareness of the importance of autopsying all young SCD victims should be increased. Ideally, these autopsies would be carried out by pathologists trained in post-mortem cardiac examination.?

The findings on arrhythmia were also new and important, said Dr Ranthe. ?Previous studies of familial aspects of arrhythmia have primarily been centred around families with very severe arrhythmic syndromes. Our finding of dramatically increased general risks of ventricular arrhythmia is therefore novel and shows that the findings from heavily affected families are relevant on a broader scale and possibly just the tip of the iceberg.?

Future work by Dr Ranthe and his colleagues at the Statens Serum Institut will focus on examining familial clustering of cardiovascular disease in new large-scale register-based studies. Their co-authors at The Heart Centre, Unit for Inherited Cardiac Diseases, Copenhagen University Hospital (Rigshospitalet) are investigating SCDs in persons older than 35 years. They will be looking at survival and disease courses in the relatives of SCD victims and examining specific subtypes of SCD more closely; those remaining unexplained after autopsy are of particular interest.
